HospiceCare provides care at home - wherever "home" may be

While HospiceCare serves most patients in their own private home, services can be provided in a variety of settings. No matter the location, the HospiceCare team works to best serve you and your loved ones and provide quality care based on your individual needs. HospiceCare is licensed to provide care in Volusia and Flagler and Orange and Osceola counties.

At Home:

If you choose to stay in your own home, we will do whatever possible to provide comfort care and to keep you home.

At Alternative Home Sites:

If you or your loved one is residing in a nursing facility, long-term care facility, assisted living, group home or other residential care facility, HospiceCare provides the same quality care and considers that location as “home.” We work closely with facility staff to coordinate the appropriate plan of care for your needs.

At the Hospital:

HospiceCare care can begin prior to discharge from a Florida Hospital facility.  We work with hospital staff to provide equipment, medications and care needs prior to your discharge for continuity and quality care with the hope of a seamless transition “back home”.

In our Volusia & Flagler County Inpatient Units -  HospiceCare at Memorial and the Stuart F. Meyer Hospice House:

There are times when a higher level of care is needed for controlling your pain or for symptom management. On these occasions, patients can be transported to our inpatient units. HospiceCare at Memorial is located on the 9th Floor of Florida Hospital Memorial Medical Center in Daytona Beach and the Stuart F. Meyer Hospice House is located on the campus of Florida Hospital Flagler. 

Continuous Care:

During periods of crisis, extended support from the HospiceCare team can be provided to patients and families on a short-term basis.  

Respite Care:

Respite care may be available for your loved one, but this service is limited.Contact our administrative office to see if you qualify.
