Our team of trained caregivers includes nurses, hospice aides, social workers, chaplains, therapists, volunteers, and physicians. Our current openings are:

Medical Social Worker - Full-Time

The Medical Social Worker is responsible for the delivery of various social work services to the HospiceCare patient and caregiver.  The purpose of the social work intervention is to provide initial psychosocial evaluations, ongoing psychosocial support, direct casework services and community education, outreach, and referral to community resources to assist with end of life needs.  Through collaboration with the HospiceCare interdisciplinary team, the Medical Social worker implements, evaluates and changes, as needed, an individualized plan of care.

The Medical Social Worker must be comfortable and proficient in addressing psychosocial and spiritual issues of end of life care including bereavement.  This staff person acts in an educational and advocacy role both in patient care and with peers and assures compliance with payor source guidelines.  Must be able to prioritize multiple requests and work independently.  Will experience periods of high stress and will need to incorporate healthy lifestyle behaviors as balance. 

Some on call and weekend work. A Master's in Social Work required and FL Licensed Clinical Social Worker preferred.  Experience working in health care or hospital settings preferred. Must be proficient with learning computer programs used in documentation.

RN CASE MANAGER - Part Time

The Hospice Registered Nurse provides skilled professional nursing care and support to the hospice patient and caregiver as prescribed by the physician and in accordance with standards of care outlined by the National Association for Hospice and Palliative Care. FL RN license required; prior hospice/case management experience is strongly preferred. 

RN HOSPICE/INPATIENT - Part Time (Volusia and Flagler Counties)

The Hospice Registered Nurse provides skilled professional nursing care to the hospice patient and family (family includes non-related caregivers) as prescribed by the physician in compliance with the regulations stated in Florida's Nurse Practice Act and standards of care outlined by the National Association for Hospice and Palliative Care. The Hospice Registered Nurse not only provides medical nursing care to alleviate physical suffering, the Hospice Registered Nurse must also assess and identify resources to provide ease of emotional and spiritual suffering to the hospice patient and family through collaboration with the hospice interdisciplinary team. FL RN license required.  Minimum 2 years Med-Surg experience required with Critical Care experiece a plus.  Inpatient Hospice experience also a plus.  Will work at both IP Units.

PRN Hospice Aide - Part Time (Volusia and Flagler Counties)

Hospice aide care in the home encompasses assisting the interdisciplinary team in providing compassionate care to medically ill patients and their families in both the home and inpatient settings. The Hospice aide is a paraprofessional member of the hospice interdisciplinary team who works under the supervision of the RN . The focus of Hospice Aide services is to meet the personal care needs of the patient and supportive care for the caregiver. In the course of providing personal care, the Hospice Aide is also responsible for observing the patient, reporting these observations to the nursing supervisor, and documenting observations and care performed as directed. FL CNA certificate required; Hospice experience strongly desired.  This is a PRN position for inpatient and home settings.

Secretary II - Full Time

Provides clerical and medical records support to the HospiceCare Performance Improvement and Education departments. Performs analytical functions associated with collecting, safeguarding, and maintaining patient health related information.  Responsible for timely and accurate data entry, routine clerical tasks, and ensuring that documents flow in a coordinated and efficient manner.  Assists in preparing and maintaining orientation schedules, materials, and related records for new employees.  Assists in the set up and operation of educational classes and seminars given throughout the year for HospiceCare employees.    Has superior computer skills and is easily adaptable to new software.  Able to manage multiple projects simultaneously, assist other departments as needed, and adhere to positive customer service principles when dealing with internal and external staff/customers. Specific duties may be distributed among other secretarial staff in the organization.

The ideal candidate will possess superior organization and computer skills and have the ability to prioritize in a fast-paced, regulated environment.  Experience in a medical setting with medical records knowledge preferred.
