Our management team holds many years of experience and an expertise which adds to our quality of care. 

	Kathleen Moore, LCSW, Assistant Director of Operations In the health care field for over thirty years, Kathleen was the first hospice social worker in the Volusia and Flagler County area. In her role as Assistant Director of Operations, she is primarily responsible for the strategic growth and development of the HospiceCare program. 	Scott Baker, RN, Assistant Director for Clinical Services A long time resident of Ormond Beach, Scott is our Assistant Director for Clinical Services and is responsible for the day to day clinical operations of HospiceCare. A veteran of the United States Air Force, he graduated from Daytona State College in 1993 with his RN degree. A member of the Florida Hospital family since 1992, Scott was the former Director of Case Management at Florida Hospital Fish Memorial. Email: scott.baker@ahss.org
	Margaret Ann (Peggy) Smith, RN, BSN, Inpatient Coordinator With Florida Hospital HospiceCare since November 2006, Peggy is in charge of both the HospiceCare at Memorial Medical Center Inpatient Unit and the Stuart F. Meyer Hospice House. Email: margaret.smith@ahss.org
	Raeanne Cade, Volunteer Services Coordinator Joining our staff in 2008, Raeanne came to us with a wealth of experience in volunteer management. At Loma Linda University Medical Center in California, she managed almost 1800 volunteers and is experienced in creating and coordinating progams and services that will benefit hospice patients and their families. 	Darlene Kimble, RN, Home Care Clinical Coordinator   With 17 years hospice experience, Darlene coordinates home patient care.  She works closely with the HospiceCare team to ensure that patients’ goals of physical, emotional and spiritual comfort are being met.  Email: darlene.kimble@ahss.org
	Leah Riddick, RN, BSN, MCEd, Performance Improvement Coordinator With over 25 years in nursing, Leah brings a wealth of diverse experience to our team.  She served over 8 years in the United States Navy in the Nurse Corps and has held positions as a utilization review coordinator, special projects coordinator and regional clinical nurse manager. She has a BSN from the University of Central Florida,a Masters in Christian Education from Dayspring Theological Seminary, and is an Ordained Minister.  Email: leah.riddick@ahss.org.
	Holly H. Van Hoose, RN, CHPN, Admissions Coordinator With over 20 years of hospice experience in our area, Holly joined HospiceCare in 2009.  Starting as a hospice volunteer in 1983, she has worked as a hospice nurse, clinical coordinator, and an admissions coordinator.  Holly has been a resident of Ormond Beach for 28 years and brings with her a passion for hospice.
