Upon admission to the program, our interdisciplinary team develops a plan of care to meet the specific needs of each patient-family unit. The professional staff is on call 24 hours
a day, seven days a week.  Our team members include:

• Physicians
• Nurses
• Social Workers
• Hospice Aides
• Chaplains
• Administrative Support Team
• Bereavement Counselors
• Other therapists as needed
• Specially trained volunteers
